Welcome to the Fox River Grove School District 3 Health Services webpage. This section of our site is intended to provide families with helpful health resources, information, and support throughout the school year. If you have any questions or concerns, please do not hesitate to contact us. As school nurses, our mission is to promote a safe, healthy, and supportive environment for all District 3 students, staff, and families.

  • State law for all Illinois school children mandates submission of these forms. Please schedule appointments early and utilize the required forms. Additional health examinations may be required when deemed necessary by school authorities. If you have questions about these requirements, please contact the school nurse at your student’s school building. More specific information about the required immunizations is on this Illinois Department of Public Health web page.

    Required Exclusion (Section 27-8.1 of the School Code): Fox River Grove School District is a first day exclusion district. Students in grades requiring submission of the Certificate of Child Health Examination form are required by law to be excluded from school if the form, or a Religious or Medical Exemption, is not received by the first day of school.

    Kindergarten

    State of Illinois Physical Examination Form entitled, Certificate of Child Health Examination, dated within one year after the first day of the previous school year. Parents complete and sign Health History Section. Complete Immunization record as outlined by the Illinois Department of Public Health.

    First Day of School

    School-Sponsored Athletics Participants May submit one of two physical exam forms:
    • State of Illinois Physical Examination Form entitled, Certificate of Child Health Examination. This form is compliant for 12 months after completion.
    • IHSA Form entitled, Pre-participation Examination. This form is compliant for 13 months after completion.
    Prior to Athletic Tryouts

    New to District 3 in Any Grade

    Don’t Wait. Vaccinate!

  • Students who will need prescription and/or non-prescription medications administered during school hours, are required to submit medications, along with a Medication Authorization Form and necessary actions plans, to the school nurse.

    Prescription medications must be brought to the school nurse in their original container, labeled appropriately by the pharmacist or licensed prescriber.  Controlled prescription medications must be brought to school by a parent or guardian.  Non-prescription medications must be brought in with the manufacturer's original label and seal, along with the ingredients listed and expiration date, and the student’s name affixed to the container.

    The Medication Authorization Form must be filled out and signed by the student’s licensed prescriber and student’s parent or guardian for ALL medications.   Students who do NOT comply with these Illinois State Board of Education requirements, will NOT be able to receive their medication(s) at school.  Whether or not your student self-carries, the necessary medication forms are required.  No medication is allowed on school property without health provider orders.

  • If your student has asthma, seizures, and/or allergies requiring emergency medication administration, the necessary action plans need to be filled out and signed by the student’s licensed prescriber and student’s parent or guardian. Students who do NOT comply with these Illinois State Board of Education requirements, will NOT be able to receive their medication(s) at school. Whether or not your student self-carries, the necessary medication forms are required. No medication is allowed on school property without health provider orders.
